If you’re unsure where to start, the Wellness Assessment offers a guided way to reflect on your current well-being — including stress, energy, nourishment, movement, and daily rhythms.
Together, we identify supportive next steps, which may include personalized supplement recommendations and ongoing 1:1 coaching, depending on what feels most helpful for you.
This health assessment includes a deep dive into your health history. The goal from this information is to create a timeline which you can use to better understand patterns and health conditions. You will also see a connection between your physical health and your mental health. I encourage you to use the information we gather to further talk to the people on your health care team. Once I receive this form and have time to review it, I will reach out and we will schedule a time to review it together.
You will leave the appointment with
- a detailed timeline which will show connections between mental and physical health
- a matrix which will show the current state of your health according to your basic health systems
- an action plan to help you make decisions on what to do next
